Unfortunately, when it comes to custom jobs, we unable to help. You need to contact the job author directly.

Please follow this guide to create custom fields you can then use in Action User:

When done, please go to EventON > Action User > Form Fields > enable previously created fields.

Shouldn’t it show the default page template from the theme?
