Categorized Menu Shortcode?

Hi, I may have alread submitted a ticket for this but its not showing up in my tickets so I am resubmitting.

I am trying to get a menu exactly like the one on this demo page but I cant seem to generate the right shortcode for it: http://demo.myfoodpress.com/categorized-menu/

How would I get that same style for my menu?
