Hi, the issue is resolved. I didn’t notice I had to select a higher number from the new dropdown on the settings page for the additional fields to appear. Thanks!

The notice on that and also you ahve set it so if a user is not logged into your site the submission form is not showing at all.

And see this, when you are loggedin the name and email values already gets filled in 🙂

So if you select name and email those fields will not show if the user is logged in because then those fields will be filled with saved user values. 

You can try this.

So basically move that line inside the if statement so that empty title values dont give that error again.

I think there is a problem with autocomplete order you’ve set for each purchase.
